At Oak Street Health, we provide Medicare-eligible adults with access to high-quality primary care. Every day, we challenge the healthcare status quo by fighting rising costs and poor health outcomes.
We measure our impact based on outcomes for our patients, the communities we serve and the U.S. healthcare system. Our 2021 Social Impact Report covers the work we’ve done across these three areas in the past year and during the COVID-19 pandemic. We’re proud of our team’s work over the past year and look forward to making an even greater impact in 2022.

The significant shortcomings of our healthcare system — poor outcomes, unhappy patients and high costs — are greatly exacerbated for older adults, especially moderate- to low-income adults who face multiple chronic illnesses.
Oak Street Health’s model was purpose-built to directly tackle all of these challenges and provide a level of care and patient experience that is unmatched by traditional healthcare providers. We are investing to bring that model to Medicare beneficiaries and dual-eligibles across the country who deserve this type of care — and in doing so, to reduce the health disparities that exist in our country.

Investing In Our Communities
We make a concerted effort to invest and reinvest dollars to uplift the neighborhoods surrounding our 130+ centers, from supporting local initiatives to the revitalization of the areas where we build our community clinics. With each center we build, we invest more than $2 million into the local community, providing both vital healthcare access and safe community spaces for seniors.
